Contact Me: A Form That Feeds Your CRM
A short form on your page, set up your way. When a visitor sends it, you get an email and the CRM gets a lead with everything they said.
What your visitors see
The default form is friendly and short. It asks for a first name, last name, email and phone, offers a topic dropdown with I need support with something, I have a question about your services and I am interested in working with you or getting a quote, and leaves room for a message. Before sending, the visitor answers a short security question. After sending, they see your thank-you message.
- Headline, intro text and the label on the send button are yours to edit.
- Topic and any question you add are saved with the submission, so you can see why someone wrote at a glance.
- On a phone the fields stack into one column and the send button spans the width.
Build the form your way
The editor lists your questions in order. Drag to reorder, rename, delete, and add a type from the list. A short form converts better than a long one, so most people keep to three or four questions.
- Question types: dropdown, multi-select, radio button, checkbox, text, text area and number.
- Required or optional, with your own placeholder text and options.
- Thank-you message with a title, body and footnote. The body can be plain text, rich text or a visual design.
- A switch for Visible to public or Hidden from public.
- Show a preferred language picker if you serve visitors in more than one language.
How you are told
A message you cannot see is a lead you lose, so Upward emails you right away. The notification names the sender and includes the topic, email, phone and message. By default it goes to your profile email. Enter another address under Send notifications to if a sales inbox or assistant should receive it. Whether the email was delivered is recorded against the submission, so a failure is never silent.
What lands in your CRM
Sending the form does three things at once: the submission is stored, a lead is created or updated, and the contact record is created or reused. The lead starts in the first stage of your pipeline and shows Contact form as its source. The timeline gets a Contact form submission note listing every field, including the page the form was sent from and the message.
- If the visitor already has an open lead, the new message is added to it. One person stays one lead.
- If the visitor arrived from a tracked link, the lead keeps that source.
- The preferred language, when asked, is saved on the lead.
- If the form was sent from a MySite, the submission is recorded on the lead or contact the MySite was made for, and the visitor receives a short confirmation that their message arrived.
Put the form on your own website
Many people already have a website. Upward lets the same form live there. In the editor, open the embed drawer, copy the snippet and paste it into your site. Choose how it appears, then tune it to match.
- Inline: the form sits in the page where you paste it.
- Popup: a button opens the form over the page.
- Tab: a small tab on the edge of the page opens it.
- Show or hide the title, intro, last name, phone, message, language and your custom questions. Choose one or two columns.
- Set colors, font, corner radius, light or dark theme and a transparent background.
- Send visitors to your own https thank-you page after submitting, and add a source tag that is saved on the lead.
- Removing the Powered by line is a paid-plan option.
Keep spam out, keep control
Every send is checked against a security question, and you can hide the module at any time. Visitor details are rendered as plain text, never as markup, in your notification email. You control who is notified, and your platform can assign the notification email template.

How Upward is different
Frequently asked questions
What does a visitor fill in?
First name, last name, email, phone and a message, plus a topic question by default and any questions you add. A security question confirms the visitor is a person. A preferred language picker appears only if you turn it on.
Where does the message go?
To your profile email by default, or to a different address you enter under Send notifications to. If your company assigns an email template for contact notifications, that template is used. Otherwise a plain message with the visitor's details is sent.
Does it create a CRM lead?
Yes. The lead has the source Contact form, the topic as its reference and a note that lists the name, email, phone, every answer, the page it came from and the message. A contact is created or reused, and a lead you already have for that person is updated instead of duplicated.
Can visitors attach files?
A file upload question is available, but on this form it records the file name only and does not store the attachment. To collect real files, use a booking question, which stores the upload, or share a file link by email.
Can I put the form on my own website?
Yes. Copy an embed snippet and choose inline, popup or tab. You can hide fields, match your colors and font, redirect to your own thank-you page and tag the source so you know which site page produced the lead.
Can I pause the form?
Yes. Set the module to Hidden from public and visitors can no longer reach the form. Submissions you already received stay in your CRM.
Do I have to answer from Upward?
No. The notification email contains the visitor's details, so you can reply from your own email, or reply to the lead from the CRM with email or SMS.
